We make sure all our new patients feel welcome, safe, and taken care of. During your first visit, our Medical Assistant or Nurse will take you in to the exam room and will input vital information such as vitals, allergies, family medical history and any medication you may be on. Once you have seen the MA or Nurse you will wait for the doctor in the waiting area to further discuss any concerns you may have.
For a shorter time in the waiting room, please print and fill out the New Patient Form and bring in with you at the time of your visit.
Please contact our office to find out if we take your specific insurance plan. Please keep in mind that even if your insurance is on the accepted insurance list we may not be in-network with your specific plan.
